Who Will You Choose? Who will speak for you if you’re unable to? Who will communicate your preferences to healthcare providers and caregivers if you can’t? Choosing a healthcare power of attorney to make medical decisions on your behalf in the event of a serious health condition, accident or emergency is an important step in planning ahead for your care.
Choose Your Person is a community education campaign sponsored by Mercy Care, Hospice of the Valley and Contexture to raise awareness of the benefits of making and documenting advance healthcare decisions.
Trained staff from Choose Your Person will be at UUCP to provide information and assistance on selecting a healthcare power of attorney and registering your advance healthcare directive with the Arizona Healthcare Directives Registry.
